The Adverse Childhood Experience (ACE) Questionnaire is a self-report measure developed to identify childhood experiences of abuse and neglect. The questionnaire was developed for the ACE study which hypothesizes that childhood trauma and stress in early life can lead to a higher risk of physical and mental health issues in later life.

While you were growing up, during your first 18 years of life:

Push, grab, slap or throw something at you?
or
Ever
hit you so hard that you had marks or were injured?

Touch or fondle you or have you touch their body in a sexual way?
or
Try to or actually have oral, anal, or vaginal sex with you?

Your family didn't look out for each other, feel close to each other, or support each other?

Did you often feel that...
